This was someone calling claiming to be an insurance company. They gave my mother a number to fax her medical card to and when I tried it failed. I called the number and they were some bogus free offer/free gift card company trying to scam your info. I called a second time and when I asked for the name of the company they wouldn't tell me and just kept asking what company I was calling. Total scam.
